Based on a thaler coin of c/1763 this medallion shows (on the obverse) the Arms of the City of Hamburg (A triple turretted gate flanked on each side by a Lion rampant regardant) above the value 48 Schill Spec - 48 Silver shillings.
At the bottom are the engraver's initial O.H.K. (Otto Heinrich Knorre 1761-1805).

The reverse depicts a view of the houses of Deichstrasse in the city from around the same date.
Below is the figure 1000 - possibly the number of medallions made.
Probably a modern striking - very sharp details
